QDRO/EDRO Required InformationTo prepare the QDRO or EDRO, we will require at least the following information, which will also assist in the prompt and efficient completion of the Order. Some of this information may already be in the Divorce Judgment, particularly in Judgments where the marriage involved minor children. We ask that you provide it separately anyway, for sake of convenience, and to assure accuracy; for example, the Judgment may no longer reflect one or both parties’ current address. As follows: 1. A complete copy of the Divorce Judgment; 2. Both parties’ full names, current addresses, dates of birth, social security numbers and phone number (s) of the individual retaining our services; 3. The date of marriage and the date of divorce; 4. If not provided in the Judgment, state specifically the plan or account to be divided. For example, “General Motors pension” is not sufficient; there are several plans. Or, there is usually a different pension for a city’s clerical workers versus police officers. If the account to be divided is an investment account, such as a 401K, the name of the investment company should be identified (e.g., Fidelity Investments, Merrill Lynch, etc.); “401K through employer XYZ” may not be sufficient. 5. If available, please provide any pamphlets or other documents describing the pension, including any sample QDROs or EDROs. For other investment accounts like 401Ks, please provide, if available, any statement from the investment company dated closest to the date of division (usually the date of divorce). 6. If applicable (Wayne and Oakland County divorces), please provide two court docket stickers. 7. Please specify if this Office should deal with the parties directly during the QDRO/EDRO process, or through their attorneys. 8. If the retirement plan to be divided is through employment with the federal government, please specify if it is a CSRS (Civil Service Retirement System) or a FERS (Federal Employees Retirement System) plan. If the employment started after 1/1/84, it is definitely a FERS plan, but prior to that date it can be either plan. 9. Our fee- generally $400.00, or as set on case by case basis, with a 25% discount for multiple orders after the first one (e.g., $700 for two orders). We do require pre-payment. Thank you,
